The Arlington Adult Day Program offers support to Arlington adults who need a safe and supervised environment during the day. The goal of the program is to improve quality of life and maintain independence of participants for as long as possible. The Program serves individuals age 18 and older in Arlington County that need a safe and supervised environment during the day. Participants may be:

• Experiencing memory loss
• Physically impaired
• In need of personal care assistance
• Limited in independent functioning

Cost

The cost will depend on income and how you pay (e.g., private pay, Medicaid, long-term care insurance). For those who qualify, a sliding fee scale is available.

Cherry Blossom PACE (Program for All-Inclusive Care for the Elderly) provides an adult day care program as well as comprehensive, coordinated care to elderly individuals allowing them to remain in their homes while receiving all necessary medical and social services. From healthcare management to transportation assistance, we strive to enhance the quality of life for our participants.

Cost

There is no cost for those who are Medicaid eligible. If you are eligible for Medicare only, there is a fee. Those without Medicaid or Medicare may pay privately.

Social workers investigate reports of abuse, neglect, or exploitation and initiate protective actions. The reporting person is not named unless (in the unlikely event) the case proceeds to court and the court chooses to release it. APS offers an individual agent to consult.

Virginia Hospital Center provides the Lifeline Medical Alert System, allowing individuals to receive immediate help in case of an emergency. Depending on needs and lifestyle, in-home units and mobile units are available. Both come with a waterproof pendent that connects to a live Lifeline associate when pushed. A fall detection pendent is also available. This service is available in Northern Virginia and North West DC Metro Region. VHC staff provides hands-on support to ensure devices function properly.

Description

The Northern Virginia Falls Prevention Alliance works to maximize independence and improve quality of life of older adults by implementing evidence-based programs focused on reducing falls and fall-related injuries. Matter of Balance is a discussion-based group intervention (which meets either once a week for eight weeks or twice a week for four weeks) that emphasizes practical strategies to reduce fear of falling and increase strength and balance. Participants learn to control falls and fear of falling, set goals to increase activity, make environmental adjustments, and exercise to increase strength and balance.

Description

The Virginia Hospital Center Safe Transition Program offers patients who are discharged from the hospital a two-month complimentary Lifeline Medical Alert System to ensure smooth transition from the hospital to their homes.

The service is free for two months after discharge from the hospital. If the patient wishes to keep the system after that time, there is a fee.

Description

SafetyNet Tracking allows family members to locate loved ones who have wandered from home and become lost. It is a specialized program for individuals who have Alzheimer's or other cognitive impairment. Individuals in the program wear a personal locator unit (PLU) on their wrist that is equipped with a tracking device. In the event of a missing loved one, the caregiver will alert the police to use the PLU to quickly track and find the missing older adult.
