POLST: Portable Medical Orders

POLST helps you to determine, discuss, and plan for your advanced care, which helps you live the best life possible. It also provides you with a medical order form that you take with you. It differs from Advanced Directives and does more than a DNR (Do Not Resuscitate order).

The Conversation Project

The Conversation Project is an initiative of the Institute for Healthcare Improvement, with the goal of helping everyone talk about their wishes for care through the end of life, so those wishes can be understood and respected. To share the way we want to live through the end of our lives. And it’s time to communicate about the kind of care we want and don't want for ourselves.
